If you would like to submit a name to the "Those Who Served" Index, please submit the following information:

For the cost of $5.00 send:  (Members receive one free listing upon submitting their membership fee)

    Branch of Service: (i.e. Army, Navy, Defense Industry)

    Your name and your relation to the person whom you are submitting:

For the cost of $10.00 send:

    Name to Submit:

    Branch of Service:  (i.e. Army, Navy, Defense Industry)

    Name of Unit or Organization:  (i.e. 1st Infantry Division, 360th Fighter Squadron, USS Arizona, Boeing Aircraft)

    Your name and your relation to the person whom you are submitting:

For the cost of $15.00 send:

    Name to Submit:

    Branch of Service:  (i.e. Army, Navy, Defense Industry)

    Name of Unit or Organization:  (i.e. 1st Infantry Division, 360th Fighter Squadron, USS Arizona, Boeing Aircraft)

    Duty or Job Specialty:  (i.e.  machine gunner, tank driver, mechanic, welder, riveter, Red Cross worker)

    Your name and your relation to the person whom you are submitting:
